Health insurance is an essential benefit to help you have quick and convenient access to healthcare when you need it. It is important to us that everyone has global coverage if possible so that no matter where you are working from, you are covered. After extensive research, we decided to partner with Aetna who we believe will provide you with an exemplary level of care.

🦷 Dental Insurance

If you (and any dependents) are enrolled in the health insurance plan, you will also be enrolled in the dental insurance plan. Maze's dental coverage will allow you to keep your teeth clean and healthy with minimal out-of-pocket expenses with no waiting period on coverage start date

💲Cost of Coverage

• Maze pays for the premium for the full-time team member's coverage at the rate equal to the Elite plan employee only option.
  • US: If you choose the HDHP to add dependents to, the Employee only plan is more affordable than the Elite plan, so Maze will cover up to the cost the Employee only Elite plan cost for the family HDHP plan.
• Maze contributes $500 monthly to your A-Maze-Ing Benefits allowance which is used for dependent healthcare costs. The intention behind the Ben monthly allowance is to contribute up to $500 towards team members’ dependent’s health plans. If you add dependents, you can expect your allowance to be reduced partially or fully based on the cost of the plan. If the cost is more than your allowance, the remaining will be deducted from payroll.
  • For those without dependents (or dependent costs are less than $500), they will receive these funds as a monthly allowance in ThanksBen.
• For part-time team members working at least 15 hours per week: Maze will premiums at the following rates: 50% for team members working 15-23 hours/week, 75% for team members working 24-29 hours/week, and 100% for team members working 30+ hours/week.
• You are able to add a spouse and/or dependents to your plan. If you want to add a dependent to your plan, you will be responsible for the premium payment. Ben is a good option to cover this expense, we can set up a monthly auto withdrawal from your allowance.

❌ Opt In/Opt-Out or 👨‍👩‍👧‍👦 Add Dependents

In Ben, you will be given the option to view the health plan details, opt in, add dependents, or opt-out of coverage.

• Your enrollment window is 14 days from your start date (the sooner the better). If you do not enroll, you will be opted out.

If you opt-out or do not enroll a dependent, you are not eligible to change this election until the next open enrollment period or if you experience a qualifying life event.

<aside> ⭐ Maze's goal is to provide equity between team members which is why we offer fully paid coverage to all team members. That being said, there is no financial benefit for team members to opt-out.

⛑Life and Disability Insurance

We know that Life and Disability Insurance is vital in ensuring you and your family have the financial support you need if something were to happen to you. The Life and Disability Plan will be there for you if you or your family ever need to take advantage of the financial benefits of these plans. Please note, you are automatically enrolled in these plans and cannot be opted out of.

While we hope the life insurance we provide never needs to be claimed, we want to make sure your wishes are followed for who will receive any payout money. To designate beneficiaries to receive life insurance payouts, please fill out the below form which is assigned to you in Sapling.

Please note: this benefit is only available for team members who work 30+ hours per week. Additionally, coverage is only available while you maintain an active work contract with Maze.
